Update Contact Information

Please search the CSL Identification number in the drop down menu.
You MUST select the ID number from the drop down menu in order for the information to populate.

Once the contact information has populated, you can update as necessary. Once the form submits, the contact information will update in the database.
If you have new students who do not yet have an ID number, please use New Student Database Entry form to enter them into the system. If you are unsure of your students' ID numbers, you can use the following link to access a form to lookup their ID: https://csl.tfaforms.net/288
