CSL ID Lookup from Email

This form is designed to allow for the selection of an email address and then the individual's CSL ID will appear. If no ID appears, then there is not an email address match within the system.

The individual's name is also shown, to allow for confirmation.

NOTE - if more than one record contain the same address, the first record matching will be displayed.

· Upon submitting the form, a copy of the information will be emailed to the address input for receipt/notification.
