NOTE: This form is not used to register Practitioner training.

Practitioner training courses are registered using the Practitioner Fee Form.

All certificated courses must be taught and registered through a CSL-affiliated community in good standing or the CSL Online Education Program. Within sixty (60) days of course completion, all students must be registered with the CSL Spiritual Development Education Department.

ALL STUDENTS who complete a CSL Certificated Course MUST be registered with CSL Home Office. Students who repeat a certificated course must be registered for review.
